STATEMENT
I confirm that Essential Living Management Ltd (ELM) staff are holding the fob/keys to my apartment at my request and I
acknowledge, provided that (ELM) operates adequate measures, this is done entirely at my own risk and that there is no liability
whatsoever on Essential Living Management Ltd, its servants or agents, whether arising from their holding the fob/keys or from
releasing them as authorised below. I confirm my instructions that the staff are only to release the fob/keys to the following
persons (I agree that the co-tenants of the apartment are also required to be listed below):

I confirm that the persons named above will be required to show photo identification (driving license, bank card, bus pass,
passport etc.) and if applicable a business/bank card or similar before my fob/keys will be released to them.
I further confirm that the staff themselves may enter my apartment to deliver packages and to make any occasional checks
necessary during periods of absence for which I shall notify them verbally.
I authorise the police or emergency services to draw fob/keys and enter my apartment in the event of an emergency or
suspicion of criminal activity.

NOTES
The access waiver is to only be completed by the lead tenant. Any tenant wishing to provide a co-tenant with access waiver
rights must provide a letter of authorisation permitting the co-tenant to exercise these rights and a copy of the tenancy
agreement.
Please do not forget to include your FULL name(s) on the access waiver form.
Even though you are the resident of the property, you will be asked to provide photo identification if in the unlikely event you
need to use the fob/keys for your property.
A brief description of the relationships that the person(s) on the waiver form has/have with you is required (e.g. cleaner/
relative/dog walker etc).
Please enter an expiry date if you wish that particular visitor to have access for a finite period only. Please enter ‘UFN’ (Until
Further Notice) if you do not wish to provide an expiry date.
If your apartment has any special features, you will need to note down these and any external contact details that may be
required. This must be updated as and when applicable.
If you wish to make any changes to your waiver, please complete a new form. Similarly, should you wish to remove someone
from your access waiver, this request should be made in writing to the residents’ team.
In the case of new requests, the completed document is to be returned to the residents’ team along with the fob/keys to the
apartment.
Failure to complete the document in full will result in it being unaccepted and returned.
No signed waiver = No fob/keys held.
Unless you have provided a signed access waiver form to Essential Living Management Ltd, we will not hold a set of your fob/
keys. Please do not leave packages containing fob/keys for the residents’ team. All fob/keys must be accounted for as part of
the access waiver system.
All fobs/keys must be returned within normal residents’ team hours.
If you wish to have a copy of your completed access waiver form, please ask the residents’ team who will oblige.
Should you wish to have an additional fob, there will be an additional charge. The residents’ team can arrange this for you and
advise what the additional charge will be.
